Hematopathology Case

 

Cold Agglutination

  • RBC clumping, ↑ MCV but corrected by warming

  • Cold agglutinin antibodies: mostly IgM. much less frequently IgG or IgA

  • Primary cold agglutinin disease:  idiopathic (no known cause)

  • Secondary cold agglutinin disease:  infectious disease (mycoplasma pneumonia, mononucleosis, mumps), lymphoproliferative disease (lymphoma, chronic lymphocytic leukemia), or connective tissue disorder (lupus)
