Atypical Lobular Hyperplasia and Lobular Carcinoma In Situ

  • Often multicentric and bilateral; not candidate for FNA; increased risk of invasive ductal carcinoma;

  • Histologically, LCIS  consists of solid nests that are completely filled and distended by small uniform cells with highg N/C ratio; adjacent ducts may be colonized by similar cells;

  • FNA usually shows low cellularity with small, poorly cohesive small groups and a few single cells;

  • Presence of cell balls corresponding to the distended acini;

  • Round to angular nuclei with mild pleomorphism and slightly irregular nuclear membrane;

  • Slight hyperchromasia with fine chromatin and prominent nucleoli.
