•50-year-old African American male. Past medical history: alcoholic cardiomyopathy, EF15-20%, CHF class II, ESRD •History of present illness: leg cramps and weakness for 2 wks, nausea and vomiting, clear cough and chills, no fever •Physical examination: diffuse abdominal tenderness, hepatomegaly, no lymphadenopathy. Serum chemistry: sodium 130, potassium 5.5, chloride 95, bicarbonate 23, BUN 67, creatinine 11.3, calcium: 15.3 | ||
