BREAST, LEFT, RADICAL MASTECTOMY:

  •  Moderately differentiated invasive ductal carcinoma, NOS type, 3.2 cm in greatest dimension.

  •  Modified Bloom Richardson: score 6 (tubule formation 2, mitoses 2, nuclear pleomorphism 2).

  •  Tumor involves dermal lymphatic vessels and extends into the dermis of the nipple.

  •  Ductal carcinoma in situ with central necrosis and calcification present

  • Axillary lymph nodes: 14/20

  • Tumor ER and PR strongly positive
